    The ACT team provides comprehensive, locally based treatment to adults with serious and persistent mental illnesses. ACT provides highly individualized services directly to consumers. ACT recipients receive the multidisciplinary, around-the-clock staffing of a psychiatric unit, but within the comfort of their own home and community. To have the competencies and skills to meet a client's multiple treatment, rehabilitation, and support needs. ACT team members are trained in the areas of psychiatry, social work, nursing, substance abuse, and vocational rehabilitation. The ACT team provides these necessary services 24-hours-a-day, seven-days-a- week, 365-days-a-year.

    SERVICE COMPONENTS

    • Psychiatric Evaluations
    • Outpatient Therapy (mental health, substance use or co-occurring)
    • Support for housing and/or employment needs
    • Medication Management
    • Case Management
    • Peer Support
    • 24 hours a day/7 day a week access to the ACT team; crisis management and safety planning; assistance with hospital diversion
    • Collaboration with inpatient and criminal justice facilities

    ACT ELIGIBILITY

    • ´Must be an adult (18+)
    • A resident of Blair, Elk, or Huntingdon County, or receiving services in the listed counties
    • Primary diagnosis of a serious and persistent mental health diagnosis

    At least TWO of the following:

    • Three in-patient psychiatric hospitalizations in the past 12 months
    • Two re-admissions to a psychiatric hospital within 30 days
    • Utilization of crisis services at least three times in any 30-day period within the past 6 months
    • Co-occurring mental illness and substance-use disorder for more than 6 months
    • High risk or involvement with the criminal justice system (within the past 6 months)
    • Homeless, risk of becoming homeless, or living in substandard housing conditions

    These individuals would NOT be considered for ACT services:

    • A non-resident of Blair, Elk, or Huntingdon County, or not receiving services in these counties
    • Primary diagnosis of Intellectual Disability or Substance Abuse Disorder
    • Current resident in a long-term psychiatric care facility or institution
